Where is our office located?

Our office is located at 311 Woodtick Rd in Wolcott, CT 06716. It is close to the end of Woodtick rd near Wolcott Police Department and Frisbee Elementary School. Click the button below to view the map and get directions to our office.

What should I bring to my appointment?

When you come for your appointment, you should bring your I.D. and insurance information. You will be asked to fill out or update medical forms depending on if you are a new or returning patient. If you wish to view or fill out these forms ahead of time, click the buttons above to view our patient forms. 

What insurance and payment options does our office accept?

At our office we accept most insurances. Call our office if you have specific questions or concerns. For payment, we accept cash, check, CareCredit, VISA, MasterCard, and Discover Card.

We offer a 10% discount to patients that pay with cash or check at the time of service.

What if I have an emergency?

If you have an emergency, and are in pain, do not hesitate to call the office. We are always willing to make arrangements to help patients that are in discomfort or pain. For emergencies please call the office at 203-879-9372 or call Dr. Dvarskas's direct phone number at 203-910-2581. You may also click the 'make appointment' button at the top of the page to schedule an emergency appointment.

What if I need to cancel my appointment?

If you need to cancel your appointment, call our office in advance to do so. We do appreciate a day of  advance with any cancellations, and there IS a missed appointment fee for any patients that do not show up without canceling. 

What if I need to know when my next appointment is?

We always remind our patients of their upcoming appointments with mailed postcards, and a phone call the day before the appointment. If you need to know when your next appointment is, just call our office and we would be happy to help.
